Studio Information & Policy Contract

I am aware that all students must be accompanied through the parking lot to and from the studio by a parent or guardian. I understand that all students must be in class, ready to dance in the proper attire listed below or they will not be permitted to participate in class. Check the boxes below to agree to the terms of the contract.

Shining Stars:

Pink leotard, pink tights, black tap shoes with Velcro/buckle straps or elastic ties and pink ballet shoes. Only fitted wrap sweaters and wrap ballet skirts are permitted to be worn over the leotard. Hair must be pulled up in a bun, tight ponytail or braid.

Rising Stars:

Light Purple or Lavender leotard, pink or tan tights, black tap shoes with Velcro/buckles or elastic ties and pink ballet shoes. Only fitted wrap sweaters and wrap ballet skirts are permitted to be worn over the leotard. Hair must be pulled up in a bun, tight ponytail or braid.

Shooting Stars:

Light Blue leotard, pink or tan tights, black tap shoes with Velcro/buckles or elastic ties and pink ballet shoes. Only fitted wrap sweaters and wrap ballet skirts are permitted to be worn over the leotard. Hair must be pulled up in a bun, a tight ponytail or braid.

Petite, Intermediate, Junior & Teen:

Plain black leotard, suntan, black or pink tights. Only fitted wrap sweaters. Skirts or black dance shorts are permitted to be worn over the leotard. Long solid color tank or fitted top with fitted black Capri or tight ankle leggings are also permitted. Hair must be pulled up in a bun, tight ponytail, or braid. Ballet: Dancers must have pink ballet shoes. Dancers that are learning contemporary and lyrical in class can also wear their ballet shoes. Jazz: Dancers must have tan split sole jazz shoes. Tap: Dancers must have black tie up/laced tap shoes.

Hip Hop:

Long solid color tank or fitted top with fitted black Capri or tight ankle leggings. Athletic wear or funky hip hop attire (no jeans). (If the dancer wears shorts that are above the knee, they must wear tights). Dancers must wear low top sneakers and they must be a separate pair of sneakers for class, not to be worn outside. Hair must be pulled up in a bun, tight ponytail or braid. Hats are not permitted.

Boy’s class attire:

Boys can wear sport shorts, sweats, or any loose fitting pants that do not go past their ankle (not jeans) with a tank top or t-shirt.

No food or drinks are permitted in the studios except for water.

Timely Payment – All tuition and costume balance payments must be paid on time. Any payments not received into the studio by the due date will be automatically charged on the following day on your backup credit card or check withdrawal account with a $10 late fee and a 3.5% service fee. Any backup credit card that is denied will incur an additional $25 fee. I also understand that Tuition is based on a yearly rate (not monthly) for 32 classes.*

Information – I understand that information is sent through email and I will read all emails/information received from Howell Dance. I will add HD as a contact in my email to avoid emails going to my spam folder.*

Doctor's Note – If a student has any type of injury that a doctor required them to not participate in class, they will need to bring in a note stating how long they are not to participate. They will also need to bring in a medical release form to Howell Dance so we can permit them to participate in class. If a dancer complains about any injuries that inhibits their full participation in class, Howell Dance reserves the right to terminate instruction until a doctor’s note is received. Howell Dance does not offer any make up time for missed classes.*

Inclement weather – The studio may not necessarily close for inclement weather, regardless if area schools are closed, since many times roads are clear and safe by the time our classes begin. If in doubt call the studio one half hour prior to your class to verify if we are open or not. You can also check out our Facebook/Instagram for closing information. We do not offer makeup classes due to inclement weather.*

Refund Policy – I understand there are absolutely no refunds or credits on registration fees, tuition, costume deposits, costume balances or tickets. If a child is dropping a class, a “class withdrawal form” must be filled out in person by the parent. Tuition responsibility will cease beginning the first month following the date of the formal withdrawal through November. After November 30th, parents are responsible for the remainder of the yearly tuition contract fees and no refunds will be issued.*

No Tolerance – Howell Dance is a no tolerance studio. Any dancer bullying or threatening another dancer will be immediately dismissed with no refunds.*

Dress Rehearsal/Recital – Each family will receive 1 dress rehearsal ticket with the option of purchasing extra. If a family has 2 or more siblings performing in our end of the year recitals, there is no guarantee that they will be performing in the same show.*

Apparel/Costume Orders – I understand that any costume or apparel must be picked up at the studio within 30 days of notification. Failure to pick up will result in it becoming the property of Howell Dance and no refunds will be given.*

Performance Participation – I am aware there is a recital performance fee to participate in recital. Families will receive a link for the digital download of the performance. I understand all dancers must order a new pair of tights for recital. Students ages 9 and above will have an additional fee for their recital bodytight.*

Consent: People Liability – I hereby release and discharge Howell Dance their owners, agents, employees and instructors, from any and all liability, claims, demands or causes of action that I may hereafter have for injuries and damages arising out of any participation or competition activities.*

Consent: Company Liability – I HEREBY WAIVE, RELEASE, AND DISCHARGE Howell Dance and all divisions thereof of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and/or instruction of camps, competitions, intensives, private instruction, choreography or any activity I may participate in.*

Consent: Likeness – I hereby grant Howell Dance Inc permission to use my child's likeness in photographs, video or other digital reproduction in any and all of its publications past, present and future, including website entries, without payment or any consideration. I understand that any photographs/videos/digital reproductions using my likeness may be used for promotional and/or informational purposes.*

Consent: I understand that only parents of children in the Shining Stars class will receive a weekly Zoom link to access their child's classes through December 13th. This link will allow them to observe their child(ren) during their class sessions.*

Consent: Security Camera – I understand that the classes are being monitored and recorded by a security webcam system, viewable by Howell Dance, to ensure the best dance education and safety for my child/children.*

Consent: Parking – I understand that parking in the fire lane is illegal and is not permitted. Parents need to park in a spot in the parking lot when waiting for their child. If this becomes a problem, Howell Dance has the right to dismiss any families that do not abide by this policy and no refunds will be issued.*

Consent: Policies – I hereby understand that failing to follow Howell Dance’s policies may result in my child(ren)’s dismissal and no refunds will be permitted.*

Consent: Contract – I understand, as parent or guardian of the above applicant/applicants, certify that I have read and received a copy of this contract. I consent to the applicant’s receiving the instruction applied for and I agree to the provisions of this contract for myself and said applicant/applicants.*

Consent: If I am a returning family at Howell Dance Company/Howell Dance and choose not to update my payment method, I authorize Howell Dance Company to continue processing Automatic Tuition Withdrawals from the account on file.
