Studio Information & Policy Contract

In consideration for being permitted to engage and receive instruction on the Howell Dance Company Competition Team, the students and parent/guardian agree as follows:

Consent: Physical Condition - I understand that the training, instruction, and/or lessons which I have registered for at Howell Dance Company/Howell Dance involves strenuous physical activity, and may have physical contact with the instructors and students. I state I have no knowledge of any physical condition that would hinder the dancer from these activities. I promise to notify Howell Dance Company/Howell Dance immediately, in writing if any such condition should develop. After review of the written notification, Howell Dance Company/Howell Dance reserves the right to terminate instruction if the review warrants such action.*

Consent: Attendance - Attendance at all scheduled classes and additional practices are mandatory. Students who miss three classes in a particular subject or come into class late more than 4 times will require a private lesson at $35 per ½ hour. Any student that comes into class more than 4 times but is unable to participate even due to sickness or injury will also be required to have a private lesson. Students that do not take their make-up classes will not be permitted to perform in any upcoming competitions or annual recital. The student or parent must notify the studio by email at least one hour prior to their scheduled class time if they are not attending class. Students missing class 2 weeks before competition will not be allowed to participate in that competition. There will be no refunds for your entry fees.*

Consent: Attire - All students must be in class, ready to dance in proper attire when dance class begins. Proper dance attire consists of the following choices:*

- Plain black leotards, suntan, black or pink tights. (Mandatory for Ballet Class)

- Skirts or black dance shorts are permitted to be worn over the leotard

- Solid color tank top, or solid color sports bra with black leggings

- Only logos allowed are HD/HDC

Hair must be pulled up against the head in a bun (Mandatory for Ballet Class), or put in a tight ponytail or braid. Side bangs must be clipped back. Any additional clothing must be taken off in the lobby prior to the beginning of class. Proper footwear is also required for all classes. Students that are not in proper attire or hair not properly placed will not be permitted to participate in class.

Consent: Cell Phones & Behavior - Cell phones/watches must be turned off during class and be placed in the unplug box. They will receive their phone/watches back at the end of their classes. If a parent needs the child you can call the studio at 732-370-5330. All Howell Dance Company Dancers and their families must show courteous and good behavior and respect when present at the studio! We will not tolerate profanity, or disrespectful attitudes or behavior toward the staff or other students. Should this arise, parents will be asked to come pick up their child from class.*

Consent: No Tolerance - Howell Dance Company is a no tolerance studio. Any dancer bullying or threatening another dancer will be immediately dismissed with no refunds.*

Consent: Medical Release - If a student has any type of injury that a doctor required them to not participate in class, they will need to bring in a note stating how long they are not to participate and then they will need to bring in a medical release form to Howell Dance Company/Howell Dance so we can permit them to participate in class. Any student with a medical release may not be permitted to compete until 2 weeks after the medical release date and after 2 weeks of classes at Howell Dance Company/Howell Dance.*

Consent: Doctor's Note - If we have a dancer complaining about any injuries that inhibits their full participation in class, Howell Dance Company/Howell Dance reserves the right to terminate instruction until a doctor’s note is received.*

Consent: Summer Classes - I am aware that there are 5 weeks of mandatory summer classes. June 8-12, June 29-July 3, July 6-10, August 3-7, August 10-13. If anyone misses any summer classes where dancers are learning new choreography, one private lesson per two classes missed must be scheduled for $35 per half hour before the fall session starts. Summer tuitiion rates are different from our yearly rate schedules. Families that have multiple dancers will receive 25% off each additional child. Dancers will be charged for their regular summer classes plus an additional fee for the master classes.*

Consent: Additional Rehearsals - Any teams that require additional practices for competition will be charged $10 per dancer per hour.*

Consent: Dress Rehearsal - I am aware that there will be a mandatory dress rehearsal February 2027*

Consent: Parent's Dress Rehearsal - I am aware that if my child is in the Ruby, Petite, Coral, Sapphire, & Pearl class that it will be mandatory for a parent to attend dress rehearsal*

Consent: Competition fees - I am aware that by being a member of the Howell Dance Company Competition Team there will be competition fees due throughout the year and they must be paid on time and are non- refundable regardless of the situation. Any competition payments not received into the studio by the due date will be automatically charged on the following day onto my backup credit card or checking account with a $10 late fee plus the 3.5% service fee.*

Consent: Payments - I am aware that all registration fees, tuition, costume deposits, costume balances, accessories and other payment related to being on the competition team must be paid on time and are not refundable for any reason. Any payments not received into the studio by the due date will be automatically charged on the following day onto my backup credit card or checking account with a $10 late fee plus the 3.5% service fee. Any back up credit cards that are declined will incur an additional $10 fee.*

Consent: Apparel/Costumes Orders - I understand that any costume or apparel must be picked up at the studio within 30 days of notification. Failure to pick up will result in it becoming the property of Howell Dance and no refunds will be given.*

Consent: Competition week - I am aware that it is mandatory for my child to attend ALL dance classes the 2 weeks before competition. There are NO EXCEPTIONS unless dancers have a doctor's note. Student’s not attending class will not be permitted to compete.*

Consent: Outside activities - Dancers participating in outside activities, like dance teams, school plays, sports, etc, and cannot commit to all rehearsals and classes 2 week prior to the competition must notify us as it will reflect their positions in their routines. (same as if they cannot commit to all competitions)*

Consent: Competition participation - I am aware that my child’s team will be competing in 3 or 4 competitions through May. I am aware it is mandatory to participate in all competitions.*

Consent: Competition schedule & awards - I am aware that Howell Dance Company/Howell Dance does not receive the exact competition schedule until one week prior to the event. I understand I will receive the schedule via email as soon as Howell Dance Company/Howell Dance receives it. I also understand it is mandatory for students to stay for awards at all the competitions.*

Consent: Competition schedule & classes - I understand that weekend competitions may occasionally replace regular dance classes. As a result, dance team classes will not be held on certain Mondays following competition weekends. Parents will be notified in advance of any Monday closures once the competition schedule is finalized.*

Consent: Competition time - I am aware that my child is required to be at the competition at the designated time, dressed and ready to perform. Students that show up late or show up unprepared will be required at the next competition to stay for the entire event. Also dance placement for future routines will be affected accordingly. If you do this more than once, your child will be dismissed from the dance team. It is the parent’s responsibility to get their own correct directions to the competition venues and to ensure your child is there dressed, ready and on time. No excuses!*

Consent: Performance Participation - I am aware there is a recital performance fee to participate in recital. Families will receive a link for the digital download of the performance.*

Consent: Recital Participation - I am aware that my child will be performing in the annual recitals in May. I am aware it is mandatory to participate in the shows.*

Consent: Private Lesson Payments - I am aware if my child signs up for a solo/duo/trio and then decides not to participate, I will be responsible for the private lesson payments to the teacher through April 30, 2027.*

Consent: Pre-Competition Private Lesson - I am aware if my child signs up for a solo/duo/trio there will be a mandatory 10 minute lesson ($10 fee per routine) with Miss April and Miss Kelly in November/December. I understand it is mandatory to attend this 10 minute lesson prior to being allowed to compete the solo/duo/trio at competition. This is in your child’s best interest.*

Consent: Solo/duo/trio fees - I am aware if my child signs up for a solo/duo/trio and then decides not to participate, I will be responsible for Competition fees for the duo/trio for all remaining competitions and I will release the duo/trio costume to the studio with no refunds or payments.*

Consent: Parent conduct - Parent conduct is also very important to instill a positive experience for our dancers. If a parent or relative has any problems, he or she agrees they will contact Miss Kelly or Miss April to professionally discuss the situation. Under no circumstances should a parent contact a competition directly. Also airing out your problems in an unprofessional manner is not only upsetting to us as a staff but also to other parents and students on the team. If things cannot be handled in a proper way your child will immediately be dismissed from the Company Team with no refunds.*

Consent: Parking - I understand that parking in the fire lane is illegal and is not permitted. Parents need to park in a spot in the parking lot when waiting for their child. If this becomes a problem, Howell Dance has the right to dismiss any families that do not abide by this policy and no refunds will be issued.*

Consent: Late pick up charge - Students must be picked up from class on time. Students not picked up within 5 minutes of class being over (especially at the end of the night), will be charged a fee of $10.00. After 15 minutes, it becomes a fee of $25.00.*

Consent: Enforced rules - These rules and regulations will be strictly enforced regardless of the dancer’s level or position in a routine. (Howell Dance Company/Howell Dance will modify the routines) Any infractions of these rules will result in the student’s dismissal from the Howell Dance Company Competition Team. There will be no refunds issued including but not limited to tuition, competition, costumes etc. (The student may remain enrolled in standard classes but will not be allowed the privilege of being a HDC Member).*

Consent: Competition Dates - I am aware that all competition dates are mandatory. I will be signing the competition schedule contract in October. By signing the contract, I will be participating and committed to the events. If for any reason a dancer cannot commit to a competition, they will be placed on the side and in the back of their dance routines so that it will not affect the other dancers. If you can not commit your child(ren) to at least 2 of the Regional competitions, they will be dismissed from the dance team. Failure to attend events after signing will result in my child's dismissal from the dance team, unless dancers have a doctor's note dated for the competition date (from a local doctor) with a legitimate illness or injury.*

Consent: Refunds - I understand there are absolutely no refunds or credits on registration/ deposit fees, tuition, costume deposits, costume balances, competition fees or tickets. Once I sign this contract, I acknowledge that I am responsible for the full yearly tuition fees for the 2026-2027 season regardless if my child participates or not.*

Consent: Promotion - I hereby grant Howell Dance Company/Howell Dance permission to use my child's likeness in photographs, video or other digital reproduction in any and all of its publications, including website entries, without payment or any consideration. I understand that any photographs/videos/ digital reproductions using my likeness may be used for promotional and/or informational purposes.*

Consent: Security Cameras - I understand that the classes are being monitored and recorded by a security webcam system, viewable by Howell Dance, to ensure the best dance education and safety for my child/children.*

Consent: Accessory, competition fees - I, also authorize Howell Dance Company/Howell Dance and/or e-online data to bill my credit card for any outstanding costume, accessory, or any competition fees not paid into the studio by the due dates with a $10.00 late fee.*

Consent: After recital - The Monday after recital will be their year end "It's A Wrap Party". From May 19th through June 1st dancers will have an abbreviated dance schedule.*
