Consent - I am aware of the nature of the programs offered by Howell Dance. I do waive, release, absolve, indemnify and agree to hold harmless the organizers, sponsors, participants and any persons involved in the operation of Howell Dance programs for any claims arising out of injury or other loss to named applicant or any member of his/her family whether as a participant in activities or as a spectator. This includes participation on any properties where students are performing.*
Consent - I hereby waive, release, and discharge Howell Dance and all divisions thereof of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and/or instruction of camps, private instruction, choreography or any activity I may participate.*
Consent - I hereby grant Howell Dance permission to use my child's likeness in photographs, video or other digital reproduction in any and all of its publications past, present and future, including website entries, without payment or any consideration. I understand that any photographs/videos/digital reproductions using my likeness may be used for promotional and/or informational purposes. Howell Dance will not release my child’s name or personal information*
Consent - I understand that the classes are being monitored and recorded by a security webcam system, viewable by Howell Dance, to ensure the best dance education and safety for my child/children.*
Consent - I understand that parents are not permitted to wait inside the dance studio during my child's class. I understand that my child will be dropped off and picked up at the studio's front door.*
